Docobo Ltd is a UK healthcare solutions provider, involved in the management and prevention of Long Term Conditions (LTC, formally known as chronic diseases). In addition to supplying a telehealth infrastructure, Docobo also provide project management solutions to enable the successful implementation and rollout of telehealth enabled services.

Docobo Ltd, is a UK company accredited to EN13485, established in 2001 from a consortium of clinicians and technologists. Docobo has been in the forefront of development of systems for the management of patients with Long Term Conditions in their own homes. A range of products and services that make up a complete telehealth infrastructure are provided including the doc@HOME® Web based clinical database for the management of chronic disease and the HealthHUB™ for collecting physiological, quality of life and life style data; data transfer and receipt of messages permit effective and efficient management of patients in their own homes.

The company is working with over 25 UK NHS PCTs and Trusts, providing the doc@HOME® remote monitoring service for LTC’s, and in many cases helping management teams to successfully implement and rollout telehealth enabled clinical services. This Telehealth service offers care providers with an integrated low cost solution for the collection, management and analysis of essential patient related data, and permits efficient interaction between clinicians and patients at home.

TeleHealth has the ability to reduce the burden to healthcare providers related to LTC management by the provision of at-home monitoring and increasingly intelligent automated management of the condition.

Service Overview

Image:Dochomeserver.jpg doc@HOME is an integrated telehealth solution for the remote management of patients with a range of Long Term Conditions (also known as Chronic Diseases). It is the means for the collection and analysis of essential patient related data, permitting effective management through efficient interaction between clinicians and patients at home.

The doc@HOME service is designed to provide a systematic approach to the management of chronic disease in the home and other locations remote from the clinicians office. Patient/Clinician interaction is typically via the Docobo HealthHub, a fit for purpose, robust, handheld data collection unit which connects through a standard telephone line at the patients home to secure server. Healthcare Professional interaction with the doc@HOME service is via secure Web access using standard browsers, enabling patient management at a range of locations. Changes in patient trends can be identified and notified to the authorised user. An authorised clinician can access the patient record on demand and send messages directly to the patient, for example, a request to visit the surgery or to change the frequency and/ or volume of medication

Overview

Purpose-designed for home use by patients of all ages with Long Term Conditions, the Docobo HealthHUB™ is an innovative European solution to the growing burden placed on health providers for managing patients at home and in the community. Similar to a PDA and using the principle of “store and forward”, vital signs measurements and changes in symptoms, side effects, life style and quality of life are securely recorded and transferred to the centre of care Image:HealthHUB.jpg

Functionality

Image:HealthHUB2.jpg The integrated display and associated functional keys enable measurement, entry and reading of information from a variety of sources. Each HealthHUB is programmed to meet individual patient needs across a range of chronic diseases. In accordance with clinical recommendations the HealthHUB can be set to monitor a range of long term conditions and co-morbidities, well-being and any combination of these. It can be easily adapted to monitor patients that require additional monitoring over time as well as those with other specific disease.

Built in sensors are used to record twenty second rhythm strips of the ECG and Bio- Impedance. Dependent variables are calculated and stored alongside physiological information derived from other optional 3rd party devices. Vital sign monitors of choice can be connected to provide automated data transfer. In addition a wide range of quantitative and qualitative information enabling continuous assessment of a patients quality of life, medication and treatment compliance is collected through the HealthHUB. The patient can track their main health parameters via the four week trend display.

The HealthHUB utilizes a “one-touch” implementation to start the monitoring process, which is simple, fast and easy to do. The patient is typically asked questions in the morning and the evening, the questions being fully customizable by the clinician managing the patient.

Feedback to the patient is via both a built-in 4 week display and a messaging capability. The 4 week display allows patients to monitor the main parameters such as weight, blood pressure, medication compliance and such like, allowing them to monitor and learn about their condition. The messaging capability enables a range of messages to be received and displayed, these messages either being automatically generated or sent from their medical team.

All data is fully encrypted before being store on the device, and is managed and controlled in accordance to the European Data Protection Directive.
